First Visit
This important complimentary 30 minutes visit to our practice will be a chance for us to  
get acquainted and find out about your specific concerns and expectation and point out  
to you any other orthodontic problems.
•   We will review your medical and dental history to assess any conditions that can
complicate treatment; therefore it is very important that we have accurate
history. After a careful examination of your face, jaw and teeth, we can
often answer general questions about treatment during the initial examination.


•   Last step would be to schedule a final consultation to discuss your particular treatment options and
estimated length of treatment.  We will also discuss your financial options.
•   If it is indicated, we will schedule a 30 minute record appointment.  It includes
x-rays, photos of your face and teeth, impressions of your teeth for
study models and are necessary for developing the appropriate
treatment plan.
•   After this visit and before your treatment is initiated, Dr. Esfandiari will send a detailed report outlining
all the dental and orthodontic findings and recommendations to your referring dentist.
•   You could greatly assist us by providing the following information at the time of your first visit:
1.    Any recent (6 months or less) Panoramic X-ray.
2.    A copy of your dental insurance card.  In most cases we can inform you of your orthodontic
       coverage during the time of your first visit.
3.    Name and information of your general dentist.
4.    A copy of your dental referral if  required by your insurance company.
